Check in before your appointment on your own device
Check in and complete many of your forms several days before your appointment using your smartphone, tablet or computer. That means fewer paper forms to fill out! When you have an appointment, expect a text message and/or email with your link to check in. Follow the instructions provided.
- Faster, smarter check-in experience, delivered via email or text message.
- Forms are completed and payments made before your appointment, on your own time, from the privacy of your own device.
- Simple and quick appointment arrival.
- If you complete the check-in in full before your appointment, you no longer need to arrive 15 minutes before your appointment time.
Are you (or someone you love) living with diabetes?
Diabetes is a disease that occurs when your blood glucose, also called blood sugar, is too high. It affects about 37 million Americans, including adults and youth. Diabetes can damage the eyes, kidneys, nerves, and heart, and is linked to some types of cancer. Working with our health care professionals who specialize in diabetes care can help you manage your diabetes.
With a passion for spreading awareness and helping our patients improve their health, we’re pleased to introduce an all new educational video series! Hosted by Heather Kleitgen, BSN, RN, CDCES, the series focuses on a variety of topics to help people with diabetes and their caregivers understand and manage their diabetes.
